Terms of Use

These terms apply when you use the PHM website and services, operated by Nutra Hero LLC, dba Peak Health Meds ("we", "us"). By using the service you agree to them.

We are not a medical provider

PHM provides technology and administrative services that connect you with independently licensed clinicians. We do not practise medicine. Clinicians make their own independent medical decisions, including whether to prescribe, and nothing on this site is medical advice.

Eligibility

You must be at least 18, located in a state where care is offered, and able to give accurate information about your health. You agree that what you tell us and your clinician is true and complete.

No guarantee of a prescription or result

Starting a visit or buying a plan does not guarantee that a clinician will prescribe any medication. Individual results vary.

Payments, subscriptions and cancelling

Plans renew automatically at the interval shown when you sign up until you cancel. You can cancel from your account or by contacting support.

Refunds

If a clinician decides that treatment is not appropriate for you, you will not be charged for medication. Fees for a completed clinical consultation are not refundable. For safety reasons, and as pharmacy rules require, medication cannot be returned or refunded once it has been dispensed.

If your order arrives damaged, incorrect or does not arrive, contact us within 14 days of the expected delivery date and we will replace it or refund the affected item. Cancelling a plan stops future charges; it does not refund charges already processed for an order that has shipped. Approved refunds are returned to the original payment method, usually within 5 to 10 business days.

Not for emergencies

The service is not for emergencies. If you think you are having a medical emergency, call 911.

Your account

Keep your login details private. You are responsible for activity on your account.

Limitation of liability

To the extent the law allows, we are not liable for indirect or consequential losses arising from your use of the service. Nothing in these terms limits liability that cannot be limited by law.

Changes

We may update these terms. The date at the top shows when they last changed.
