Telehealth Consent
Last updated September 28, 2026
Telehealth means receiving medical care from a clinician using electronic communication, such as secure messaging, questionnaires and video, instead of in person. Please read this before starting a visit.
What to expect
- A licensed clinician will review the information you provide and may ask follow-up questions.
- The clinician decides whether telehealth is appropriate for your condition and whether to prescribe.
- You may be referred to in-person care if your needs cannot be met safely online.
Benefits and risks
Telehealth can make care more convenient and quicker to access. Its limits include that a clinician cannot physically examine you, and that decisions depend on the accuracy and completeness of the information you give. Technical problems may delay care.
Your rights
You may decline telehealth and seek in-person care at any time. Your information is kept confidential as described in our Privacy Policy.
Your agreement
By starting a visit, you confirm that you have read this consent, that the information you provide will be accurate, and that you agree to receive care through telehealth.
